Default Best shirt-pocket notebook?

The Moleskines I've seen are too large; the 3x4.25 Clairefontaines I
just received are rather small. Suggestions for a good ink-friendly
notebook that will fit nicely in an ordinary shirt pocket?

I share your pain. I don't know any to recommend. Years ago I
discovered a felxible but strong covered notebook, pocket size, where
the all the pages were simply folded over at the top and stitched with
thread.
Found it at Sears Roebuck, of all places! Went back and got a salesman
to sell me a case of them - used them for years. He said this was a
style favored by WWII miliary officers, because they fit in the uniform
jacket breast pocket. ANd back then ballpoints were not available,
only recently invented in Germany, so FP ink worked well in them.

SInce then, the closest is the drugstore top or side spiral bound, with
really poor paper.
I gave up.

I have since made my own pocket "notebooks" -- I buy 80 lb cover stock
paper, cut on a paper cutter (some waste), and fold a bunch of the
sheets booklet style along the side. No stitching or stapling. The
advantage is I can remove and save used sheets, and add blank sheets
every day. The heavy paper makes up for lack of a cover.

The same paper I also just fold the 8.5 x 11 sheets and tuck several
"packs" in a briefcase.

Advantage: You can use whatever paper you like best.

Why don't you buy some index cards and binder clips and create so called
"Hipster PDA" for yourself?

Have you tried Ampad's Gold Fibre version? A number of people here
and other places have recommended Gold Fibre pads and spiral bound
notebooks. For inexpensive paper, I've found they work pretty well
with FPs. My briefcase always has a Gold Fibre Memo Book, a
Clairefontaine notebook, and Levenger note cards.

The Gold Fibre Memo Book is 3" x 5" with the spiral on top. Their
Designer notebook, with spiral on the side, measures 5" x 7" w/o the
spiral.

I have used both a Rhodia 12200 3 3/8 x 4.75 inch 80 pages of graph paper
bound in a pad form, really, really nice paper and the pages tear out. Also
a moleskin volant notebook 3.5 x 5 inch with 32 pages of lined paper, don't
like the paper quite as much. It is bound in stiff, heavy weight brown paper
and the last 16 pages are meant to tear out. It also has a pocket in the
back cover. It is thinner than the Rhodia. Not fancy but they are cheap and
fit in a shirt pocket quite well.

Second the Rhodia, I use the 11200, 2.9" X 4.1", 80 sheets. Perfect for a
shirt pocket and has a stiffened back board.

Rhodia #13 (around 4 by 6 inches) is also good if you have big pockets.

Folded A4 sheets.

That's what I use; shirt pocket, jeans pocket, whatever, will fit in
very nicely.

And if you've got some scissors handy, why not jazz it up a little and
fold'n'cut the sheet into an eight-page minibook?

That last link makes absolutely no sense in its directions.
I think, instead: Fold a letter size paper in quarters (or just cut in
quarters, why waste folding). Stack the resulting quarters together,
and then fold them all together in half.
